The Initial Phone Call
ComForcare’s well-trained intake staff is prepared to take information about your needs and answer your questions.

Once we understand your specific situation, we will discuss options available to your loved one, describe our services in greater detail and schedule a no-cost, no-obligation in-home visit to conduct a more thorough needs assessment.

An In-Home Visit with You, Your Loved One and Family Members
We offer families the opportunity to get to know us better during a no-cost, no-obligation, in-home visit with one of our staff members. This visit can take place in a private residence, skilled-care facility, hospital or rehabilitation center.

During this visit, we will further evaluate the needs of your loved one and your family, set goals for care, perform fall prevention and home safety checks and discuss costs and available funding options.

In-Home Nurse Assessment
Once all parties agree on an appropriate level of care and to begin home care services, one of our experienced staff nurses will visit to perform a complete assessment of the client’s physical, mental and social needs.

The nurse will then develop a personalized care plan, based on the assessment, that will be followed by the caregiver at every visit.

Caregiver Selection
We want your home care experience to be successful. Key to that success is the development of a warm and trusting relationship between your loved one and his or her caregiver or caregiver team.

We consider what we have learned about your loved one during our conversations, visit and assessment and then carefully match your loved one with a caregiver or caregiver team that best matches his or her personality and needs.

Client Record Management and Scheduling
Throughout the process, we record your loved one’s assessment and care needs into our proprietary software.

This enables us to match your loved one with an appropriate caregiver or caregiver team, schedule visits according to your needs and monitor each shift for appropriate coverage.

Follow Up, Quality Checks and Reassessments
To ensure that your loved one is receiving the highest quality of care, we will call you or your loved one at the end of the first shift, the first week, the second week, the first month and every month thereafter.

We also perform in-home supervisory visits of our caregivers and reassessments of your loved one to ensure their needs are being met, making adjustments to the care plan as needed.

Exceptional Care
We are available 24/7/365 and employ a large staff of caregivers to guarantee uninterrupted service.

We provide on-going training and education for our caregivers so they are the most knowledgeable in all areas of home care.

If your loved one’s needs change and extend outside the scope of our care, we will coordinate resources to meet those needs.
